Well Visits
At Kids Plus, we don’t just help your children when they’re sick; we help them, and your whole family, stay well all the time.
The most important way we do this is to see you for regular Well Child Visits. We’ll monitor your child’s growth and development, answer your questions, address your concerns, and provide critical vaccinations that save lives and prevent serious illnesses.
Well Child Handouts
For each visit, we provide handouts with information specific to your child’s age and stage of development. We’ll deliver them straight to your Patient Portal account, or you can access them here; just click the links below.
Pre-Visit Questionnaires
We also have Pre-Visit Questionnaires that you can fill out before you come to the office, to help make your visit even smoother and more efficient. PVQs allow you to give us the info we need, and share the concerns you have, ahead of time. Links are below; just click the one for your upcoming visit, and we’ll be ready to go when you arrive.
